TDAP Vaccination

Attention ALL Parents and Guardians:

A new California law requires students to be immunized against Pertussis, better known as Whooping Cough. This law requires all students entering 7th through 12th grades to provide proof of receiving a Tdap booster shot BEFORE starting school August 1, 2011. Please take your student to his/her doctor to receive this important and necessary shot.

You may receive the shot at the Donna Leffler Health Clinic {909-820-7785 ext. 140} if you meet all of the following criteria:

1. Birth to 18 years of age
2. Eligible for Medi-Cal or CHDP
3. Have no health insurance (Students covered by Healthy Family are not eligible).

Students return to school on August 3, 2011. Please plan to have your student ready for school by this date.
