Gather the necessary identification, residency, and health records listed below before completing your registration.
1. Identification & Proof of Residency
Valid photo ID of the parent or legal guardian enrolling the child.
View acceptable proof of age documents
- Birth Certificate (required for preschool)
- Certified Birth Record
- Passport
- Baptismal Certificate
- Official Affidavit
⚠️ Must be dated within the last 30 days.
View acceptable proof of address documents
- Utility service contract, statement, or payment receipt
- Rental property contract, lease, or payment receipt
- Property tax payment receipt
- Pay stub
- Voter registration confirmation
- Official correspondence from a government agency
- Declaration or affidavit of residency
2. Health & Immunization Records
Required immunizations list
- Diphtheria, Pertussis, and Tetanus (DPT)
- Polio
- Measles, Mumps, and Rubella (MMR)
- Hepatitis B
- Varicella (Chickenpox)
- 7th Grade Entry Requirement: Tdap booster
- TB Risk Assessment or Test Results
3. Additional Documents (If Applicable)
Unofficial transcript or recent report card from previous school.
Current or most recent signed IEP or 504 Plan (if applicable).
